CCF offers a several programs tailored to teens and young adults, including a Facebook discussion group, buddy matching program and Youth Ambassadors.
CCF Youth Connect
A closed Facebook discussion group for youth 12 and older, CCF Youth Connect provides a safe place for diagnosed youth/teens to ask questions, share experiences and offer support to others with similar experiences. Youth Blogger of the Month
Each month, a different youth member is highlighted on CCF’s Youth Connect page. The guest blogger shares weekly posts of their experience with cardiomyopathy, coping strategies, support systems and success stories. If you are interested in being a guest blogger, email Shari Maurer.
Heart Buddy Program
CCF’s Heart Buddy Program connects pre-teens and teens with each other through various communication channels (phone, email, text, Skype, Face Time, Facebook). The matching program is designed to provide support and friendship among CCF youth members as they approach adulthood. Download a CCF buddy matching form.
Transitioning to Adult Care Toolkit for Teens
CCF offers a toolkit for teenagers with cardiomyopathy to assist in making the transition from pediatric to adult care. The toolkit offers practical guidelines, a readiness assessment, medical summary form and a sample transition plan to ensure a smooth transition. Download Transitioning to Adult Care toolkit.
